Documents required for Commercial Vehicles Own Damage (OD) Partial Loss Insurance Claims - this is a list of Documents required at the beginning for surveyor appointment -(at the time of claim initiation)

1. Insurance Claim Intimation letter - immediately after accident

2. Current Insurance Policy Copy

3. Duly filled in Motor Insurance Claim Form - signed by Insured (In case insured is other than Individual, claim form has to be signed with Company Stamp / Seal)

2. Written statement from driver of vehicle at the time of Theft

3. Final Police Report (Vehicle Non Traceable Certificate) Or Charge Sheet in case of suspects identified / arrested

3. Disability Certificate with Injury Report.
4. Details of Hospitalization with details of treatment.
